As was said in there, the judges will be the one to decide whether that ninja did qualify in terms of skills, tactics, technics and whole lot of other things. As for example with Shikamaru which is the only Genin to pass the Chunin exam but he didn't win the game or rather he gave up. He qualify to become a Chunin since the judges seen some qualities of a good Chunin.
Because the contestants in the exams were not yet able to advance to become Chunin until the Hokage personally promoted them. After the fight, they still had to go through the whole promotion process- they can't just be Chunin if they one the final battle. Also, when everyone else became Cunnin or higher (in Shippuden) Naruto wasn't able to be Chunin since he was training away from the village with Jiraiya. Naruto knowing he is strong they didn't want to make him a chunin or jonin yet. They didn't think he was ready. Because he would just run into battles, and they want him to get wiser and not run into battles without thinking about the apponnents strength are let itachi get his hands on him.

Not really. Although he did beat Neji, the "judges" didn't think that he was good enough to be a Chunin and the Chunin exams got interrupted as well.
