Sailor Cosmos only appears definitively in the Sailor Moon manga, and there, her identity isn't definite. She initially appears as Chibi-Chibi, and later as Sailor Chibi Chibi, before revealing her identity as Cosmos in Act 51 (Stars 9). Sailor Ceres of the Sailor Quartet stated that Cosmos must be the "ultimate future form of Sailor Moon," but that statement in itself is ambiguous, as it could refer to a descendant of Sailor Moon (similar to Sailor Chibi Moon), a future incarnation of Sailor Moon (much like Princess Serenity is a past incarnation), a Sailor Moon from an alternate future, or a new form of Sailor Moon following her known future form as Neo-Queen Serenity. So it's possible that Sailor Cosmos' name is actually Usagi Tsukino, but she comes from a future that is so ravaged by war that identities don't matter anymore.

In the animated series, Sailor Moon has three distinct, separately-named forms: her original, which we see through Sailor Moon seasons 1-3, Super Sailor Moon, which we see in part of Season 3 and 4, and Eternal Sailor Moon, which we see in part of Season 4 and Season 5. In the manga, a possible fourth form, separate from the "Sailor Moon" lineage, is Sailor Cosmos. Her origins are intentionally vague, even when series creator Takeuchi was asked about it in interviews; Chibiusa's Senshi Team from the future, the former Amazon Quartet, see Sailor Chibi-Chibi transform into this Sailor Cosmos, and ask if she is "the future Sailor Moon." Cosmos refers to herself as "the me from this time," and occasionally will use the third-person (Usagi, Eternal Sailor Moon)--not as one would if she were referring merely to her past self (me, the other me, my younger self, etc). Furthermore, Cosmos doesn't undergo any harmful effects (such as the duplication paradox Sailor Moon and Neo Queen Serenity were warned of in their near-meeting toward the end of the Black Moon arc in the manga), which implies Cosmos, while she may be related to Usagi, Sailor Moon, or Serenity in some way, is not actually the same physical body that Usagi is. It is worth noting that in the anime, Chibi-Chibi has an entirely different origin, although a shadowy form that resembles Sailor Cosmos was shown in the last episode. No. In the animated series, Chibi Chibi (Chibi Chibi Moon when she transforms) is the physical manifestation of the "Light of Hope," and Sailor Galaxia's Star Seed. She transforms into a sword to enable Princess Serenity to battle Sailor Galaxia when she is fully transformed by Chaos, but Princess Serenity chooses not to use the sword to fight Galaxia. Galaxia shatters the sword with her own, causing it to revert back into the seemingly-lifeless body of Sailor Chibi Chibi Moon. However, after Sailor Moon has defeated Galaxia and Chaos within her, Chibi Chibi reappears and thanks Sailor Moon.In the manga, Chibi Chibi's origins are different: she is actually the disguised version of Sailor Cosmos. She was from a devastated future where Chaos fully gained control over Galaxia and became Sailor Chaos, destroying everything that Cosmos held dear. Sailor Cosmos never states what, if any, "real" name she had at any point during her appearance in the final acts of the Sailor Moon manga. However, she does reveal that she took on the form of Chibi Chibi in an effort to encourage Sailor Moon/Usagi to destroy the Galaxy Cauldron. Sailor Ceres states that Cosmos is the "ultimate form of Sailor Moon," but it is believed that this is intentionally vague wording on mangaka Naoko Takeuchi's part, as there's no way to determine if Ceres was referencing Usagi as Sailor Moon, the title of Sailor Moon (which can be passed down to daughters, as Usagi inevitably would do for her daughter, Chibiusa), or something else, such as a future incarnation.
