Basically, it's about a girl who's in a relationship with a guy who treats her badly and she feels really vulnerable and hurt because of it.
To be more specific, she starts out the song mentioning a time when someone gave her advice about love:
"Someone told me I should take/caution when it comes to love"
But because of several factors - "you were strong and I was not... I was careless, I forgot" - she didn't take caution and jumped into a relationship despite warning signs about the guy.
Now she is speaking after the end of a tumultuous relationship - "Tell them I was happy/and my heart is broken" - and this is when she feels vulnerable and weak - "All my scars are open", "And if you're done with embarrassing me..." because she feels foolish for ever loving a guy who treated her so badly.
If you haven't watched the video you should cuz it would explain things more!

To give one's all in spite of overwhelming odd and public derision, to reach for the goal that everyone says is impossible and/or ridiculous. To stand for something instead of falling for anything. To stand for the right when all around champion the wrong. This is part of what it means....
