Horvath survived the attack, he was simply knocked unconcious. It's assumed he ran away, because after the fight with Morgana, Balthazar only found Horvath's hat. At the very end of the movie, after the credits, Horvath's hat is shown sitting on a table (near the original sorcerer's apprentice hat that Micky Mouse wore) in what is supposedly Balthazar's Arcana store, and we see a hand with a figurine ring (assumedly a sorcerer's ring) pick up the hat, and the shadow of the person walking away is shown with a cane like the one Horvath used. This was probably meant to set up a sequel to the movie in case one was made.

The stage was set for a sequel when Dave picked up his hat and hung it on the lamp post. If you watch past the credits, there's a scene where what looks like Horvath picks up his hat and puts it on. However, the possibility of a sequel for a movie usually depends on how it does in theaters, not on what cliffhangers it leaves. Film makers are more interested in potential profit then how the story is left. The Sorcerer's Apprentice did poorly in theaters, yet i've hear the video sales were moderately well once it hit dvd and BluRay. In the end, it's the sole discretion of the Disney company whether or not we will be seeing a SA2 in theaters anytime soon if ever however because of the poor ticket sales in theaters, I doubt there will be a sequel.
