There is no MPAA "D" rating.

There are: G, PG, PG-13, R, and NC-17.

'D' may be used in television or in other countries.

I believe the answer is drugs or drug use. 2011 Movie "Hobo with a shotgun" has a D rating included. There was quite a bit of Deviant behavior in the movie with prostitutes and children. Many main stream movies with all kinds of drug use have no D rating included.

L means language

V means violence

S means sex

X means extreme violence and sex; only for adults it is an older version of NC-17

FV means fantasy violence

N means nutity or nakedness

In the United States, there is no D rating for theatrical films. Television does use it for Dialogue.
