It can mean anything from sexual content to violence. Take for example, Saw, the disturbing images would be watching someone get tortured before killed. Disturbing images with sexual content could mean someone being tortured while naked or something being done to someone sexually that you wouldn't do in normal, everyday life.

A disturbing image is something that could possibly put another at unease. So when movies are rated higher for "disturbing images" it just means that it will probably cause you some anxiety.
