He was technically dead fo over 1 minute. He now needs a Cain to walk. Dr. Houses leg had mussle death due to an infarction during surgery. He was going to have it amputated but stibornly refused to have his leg emoved despite the large amount of pain he was in. He went into complex white attack acardia and was technichally dead for over a minute. He then asked to be put in a chemicaly induced coma. They (the doctors) went in surgicly and removed the dead mussle. This was all when he was still with Stacy. In the end, his leg was ok and alive but he walks with a limp for all his life and is constintly in pain. He's addicted to pain pills, Vicodin, and in series 7 he went to rehab. He is now off Vicodin.

He revealed all this in series 1 episode 21 'three stories' during a medical lesson to students.

Five years before season 1 took place, House was golfing with his at-the-time girlfriend, Stacy, when blood clotted in his leg. It led to an infarction and muscle death, but was misdiagnosed as drug-seeking behaviour. Because of the mistake, House had two choices -- either live a live full of excruciating pain and possibly die, or amputate. House wanted to keep his leg, so he had Cuddy, his physician, induce a coma so he could sleep through the worst. At this time, Stacy, his medical proxy, tells Cuddy to do a "middle ground" surgery -- remove part of the leg, but not all of it. He has chronic pain because of this, but less than he would have had otherwise. He resented Stacy for backstabbing him like this and they separated shortly after. In the season finale of season 2, a man comes into House's office and shoots him twice: once in the neck, once in the stomach. He never intended to kill House, only to make him "suffer". He wanted payback because his wife was treated by House and while investigating her illness, House asked if either of them had had an affair. The husband admitted to it, but it ended up being irrelevant to her illness (genetic predisposition to brain anneurisms). House told her about the affair anyway, and she then killed herself.

An aneurysm in his thigh had clotted, went undiagnosed for 3 days leading to an infarction and causing his quadricep muscles to become necrotic. This is explained in S01 E21 titled "3 stories"
