Sequels have existed going back at least to the 1930's. There were more than 12 sequels to Mickey Rooney's "Andy Hardy" films. But the first sequel to use a number in the title was "The Godfather: Part II" (1974). It is even more commonplace today, seen as a safer bet for studios.

Two. "The Godfather Part II" (1974) and "The Lord of the Rings: The Return of the King" (2003).

"The Godfather Part II" is the only sequel to a Best Picture winner -- "The Godfather" won the honor for 1972 -- to win Best Picture itself. "The Return of the King" was the third consecutive "Lord of the Rings" installment to be nominated for Best Picture, but it was the only one to win the award.
