It's a very bad idea for starters. However, in simple terms a tracheotomy allows for long term ventilation and to prevent aspiration which is swallowing food or water into the lungs. Essentially it closes off the trachea (windpipe) from the esophagus (throat). Smoking is possible because of this simple design
