i am not sure...this may or may not be the reason....

It might have been a surgery of the lower mandible due to some complications in childhood.

i am not a surgeon or anything, but i can speculate since i had a similar surgery in the same region due to some extra growth of bone when i was young...

AGAIN, this is just a speculation.

[Googling reveals that Pickering has burn scars on his arm/shoulder... perhaps they extended higher... or perhaps it's something to do with skin grafts.]
