Generally speaking, the more Music Theory knowledge you have the better. It is not necessary to understand music theory to play music. For example, you can sing or pound out notes on a piano without any music theory training. It is possible to learn an instrument with only a limited amount as well. However, usually note reading, chords and chord progressions, major and minor scales, modes, intervals, etc. are critical for improving as a musician. While these skills do not necessarily need to be learned in a music theory setting, they will likely be gained as a process of improving as a musician. Studying music theory itself is likely to speed up that process and allow faster growth as a musician.
No. I play piano and, well, I do know all of the major chords but you don't need to. All you need is to know how to read music, or you can finger pick (which is what I do lol but I can play pretty good finger picking ^_^).
