Any time you rear end someone, you are at fault. You are still at fault, you just got lucky that the person you hit left.

The car who hit you is at fault , however if you did not have your brakes applied , and if having them applied would have prevented you from hitting the car ahead of you , you would be partly to blame for hitting the car ahead of you. That issue depends on the speed that the other car hit you and the distance you were from the car ahead of you

If you were in the middle of the intersection it is your fault as you are not legally allowed to enter an intersection unless you have the time to safely complete the turn.

that would be the person that goes into reverse, because as long as you are stopped it is not your fault. the only possible chance of it being your fault is if you are in motion, but even than it would be a technicality that you could be at fault.
It must be the rear brakes at fault.
