You don[t say what kind of car you are working on. You may have to pull the radiator to put it back in. Sometimes it is easier to remove the lower hose to drain the radiator.

Both. The reservoir is the overflow. But in the event your radiator is low, your reservoir will back it up.
i used to work in a radiator shop back in the day and we put a radiator on a rotating machine and put muric acid in it and let it rotate for some time, then flushed it out and the radiator was totally clean, no more rodding. they were copper then. not sure about todays alum radiators
unhook the lower radiator hose from the engine, and there it is... you just pull it out, put the new one in, hook your radiator hose back in and your done.
Most definitely. A blown head gasket can cause the temperature to rise and put back pressure into the radiator, therefore possibly causing the radiator hoses to blow, or even bursting the radiator.
Put it back into your radiator. You have a head/ headgasket failure. You will need repair.

It doesn't matter which way the lines go back on as long as your not putting too much stress on them. The transmission fluid can flow through the radiator in either direction.
take out the hose out of the radiator. then put a hose in the radiator hose, and let the car run with the heat on. it will take about 10 minutes. the coolant will be coming out of the radiator. then once the radiatior is leaking clear, you can put the hose back on, because it is flushed out.
