That could be a number of things. It could be your TPS. Your timing could be a little off. There could be a vacuum leak somewhere. Your EGR could be bad. Your injectors could be dirty. Your idle speed could be set incorrectly. Your fuel pressure regulator could be operating at a low pressure.

could be alot of things from a vacuum leak to a IAC (idle air control) module the type of car is very helpful
There are many causes to low heart rate in unborn babies. One of the causes could be not enough oxygen to the baby.
The idle speed is factory pre-set, and no adjustment is available. If the idle is too high, or too low, you will be having other problems that will affect the idle. In cooler temperatures, it will take the engine longer to come out of fast idle, as the engine warms up to operating temp. If the idle is too high/stays at a higher than normal idle, then take the thing to get it repaired. Running a car in gear(automatic transmission) with a higher than normal idle, can damage the transmission.
As far as I've been told, the idle is set by the pump? So if it's low the pump might need rebuilding soon.
Low oil level or pressure Hydraulic lifter bad
Usually some carbon deposits are left clinging to the combustion chamber surfaces, these deposits are red hot and ignite fuel/air mixture after the ignition is turned off. This could be caused by letting the vehicle idle excessivly long or only using it for short low speed trips. some times it may be caused by too rich of a fuel/air mixture. If it is carburated, if the idle is too fast it will cause it to diesel.
