Probably because it takes a long time and a lot of dedication to perfect the art. Also, it costs more on average to buy all the equipment and pay the classes than other art forms, especially when you get onto pointe. Ballet requires performance, stamina, flexibility, determination and technical skill and flair. You are required to master different end of the spectrum and you need to be able to know and use your whole body to help you.

There is no particular level. :) Just make sure you take at least 2 years of ballet before you build the muscle memory and experience to advance to that level.

You could but if your ballet teacher has instructed you to. You have to at least practice ballet 3 times a week for a certain amount of years so that you will be strong enough to go on pointe.
No, 15 isn't too old to start/re-start but its more that it is easier when you have started ballet at a young age because your body is more attuned to the physical pressures of ballet. But if you have done ballet it shouldn't be too bad and at least you'll have some sort of idea of what you are doing! :D
