It's probably WEBBANK/BML, which is associated with PayPal. It's supposedly connected to the Bill Me Later (BML) feature, but I got an inquiry on my credit report even though I've never used BML. I use Paypal very rarely.
If you have a Paypal account, check to make sure no unauthorized purchases were made using it. If you don't have an account, check your bank account and then contact Paypal and find out if someone fraudulently opened an account in your name. If they did, you also need to contact the credit reporting companies (Equifax, TransUnion, etc.), report the fraud, and request that the inquiry be removed from your credit report.

An insurance company can assign fault regardless if a police report is filed or not. A police report is simply a report made by a neutral party at an accident scene. I believe there have been cases where insurance companies have assigned fault to one party when the opposing party was initially named at fault in a report.
The PO Box is for a company named CPU/CBSD, which is fraudulent Home Depot/City Bank card payment center. They claim a false account in your name/Social Security Number that is a reported scam. It's more or less legit, it's just not free. The only way to access a free credit report is a site called 'annual credit report . com' (all one word, obvs). Make sure to type it in correctly; similarly worded/named sites all charge money.
