By designer are you asking what Model it is? If so, the Model can be determined by the serial number, which is located on the barrel flats and the forend. You have to take the barrel off the receiver to see them.
That is a Stevens double. Pull straight down on the forend to separate it from the barrel set. Then open the barrels and remove them down and forward. Far as you need to go.

The gun was built by Savage/Stevens for Sears, and is basically the Model 311. There were several different types of forend used on this model. We can probably supply the part, need to know if you require only the wood or the entire assembly including the forend iron. repair@countrygunsmith.net
It's on the barrel under the forend. You'll have to remove the forend.
Remove the forend, then the barrel. The serial number will be on the underneith side of the bottom barrel at the point where it hinges with the rear stock. You may need a magnifying glass.
