All depends, where you live, what is your relation to the boy and how much you can afford.

Mobile phone if he doesn't have one. Or perhaps a gaming console like PSP, Xbox360, WI or PS3. If looking to buy a real surprise; buy him a laptop.

You can get him an RC helicopter or plane if you live in an open area or get him an RC car. If you are giving him any electronics; do give him rechargeable batteries with the recharger.
