STOP messing with it immediately. Only your service provider can provide you with your PUK. It is not on your box and it is not on the card that the SIM card was punched out of like a lot of people on here will try and tell you. The PUK is unique to each SIM card so nobody on here can give you one (well, I guess they can but it will not work) and if you put in an incorrect code 10 times you will "fry" the SIM card and will have to purchase a new one for around $25-$30. Call your service provider and explain that your phone is asking for the PUK and they will give it to you and will walk you through putting it in and getting your phone working again. I do know that AT&T also allows you to get your PUK on their website but I do not know if others do that or not.
