When a large air bubble gets into the grease gun's piston cylinder, your grease gun is now one of the world's smallest and most inefficient air pump. The remedy for this problem is the little button on the front cap of the grease gun next to the handle pivot or piston. It takes much more air pressure than the grease gun's piston and cylinder can create to move any grease with this small air bubble. Just imagine squeezing a car tire to the size of a doughnut with out popping it. Relieve the pressure of this air bubble and now you can vacuum in some grease behind it with the grease gun's piston and cylinder repriming the grease gun. Long answer short: airbubble.

The grease guns I'm familiar with don't get "filled". The cartridge is just inserted into the gun; if it's not intuitively obvious to you how to do this you probably don't have the mechanical aptitude to be using a grease gun anyway.
