The best jewelry to use for them is 1/2" long 14g barbells that are internally threaded. If these are being bothersome, move over to PTFE barbells. Some people prefer them due to their bendiness, but it is for that reason that some don't like them too.

That probably refers to the ratio between the length of the ladder, and the distance at which you place the bottom part of the ladder from the wall. If this distance is too short, you have the risk of the ladder falling backwards.
