The United States hasn't tested a nuclear device for many years, but in the past each bomb and its associated test both had their own independent code names, and later a codename for each weapon and a test series. While many milestone tests and bombs' codenames have been declassified, many others have not.
A good source for some of the bigger key tests is the movie "Trinity & Beyond: The Atomic Bomb Movie". It was released on DVD several years ago, and is an excellent documentary on most of the key weapons tests performed by the U.S., Russian and China.. As one who worked with nuclear weapons in the Navy, you don't really get an idea of the scale of devastation until you see the test blasts on film. The best single source of codenames for US tests, series, devices, components, etc. is Chuck Hansen's work Swords of Armageddon. You can either buy it directly on the net or have your library get it for you on interlibrary loan.

If you mean accident with nuclear weapons, the US codename for that is Broken Arrow.One example is the incident at Palomares Spain, where a US Bomber carrying 4 hydrogen bombs collided with a refueling tanker. Both planes crashed and the unarmed bombes fell on the Spanish coast. 2 landed inland with physical damage but no explosion, 1 landed inland with chemical explosion in primary on impact, and 1 landed in the water and remained lost for a few months until located by divers.Another example iis the Thule Greenland accident, where a B-52 with unarmed hydrogen bombs had an onboard electrical fire and the crew bailed out. The plane then crashed on the ice and at least one bomb had a chemical explosion in its primary.
The cold war is over but Russia may attack us with nuclear weapons. Get in a bomb shelter and pray like hell you won't turn into some fugluy creature.

as of 1992 there is no testing of nuclear explosives in the US. that is now only done by computer simulation on very expensive supercomputers. the nonexplosive parts of nuclear warheads do still undergo both lab tests and test missile launches.
