It's a piece of metal shaped kinda like a labret post. It has a flat end and a decorative one. The end could be a cone, dome, ball,star, or something else. A slit or pocket is made under your skin and the flat part/anchor of the jewelery is placed in there. Some skin divers have holes in the anchor for skin to grow through and keep the piercing in. If they don't have holes, they aren't worth it cause they'll rip out easily. Skin divers can't be changed whereas the ends of micro dermals and be unscrewed and replaced with a different shape/gem.

Actually, it is more a problem of how much skin is available in the piercing area, and how thick that skin is. Thin skin is more likely to allow the piercing to eject. Check with a licensed body piercer and let them assess your navel skin where the piercing would be.
