If a microdermal piercing falls out this is usually due to the body rejecting it due to it being knocked constantly or when the piercer did it, it wasn't put in deep enough. Microdermal piercings are meant to be permanent hence why they need to be cut out.
You need to wait for the skin to heal and then you are able to get it put it back in, but only some piercers will put it back in so be careful.

I'm pretty sure you'd need to get the tattoo then the piercing, as it could be really hard for the tattooist to work around the dermal, especially if it's new.
