The use a needle..just like with any other pericing. They clean you up with alcohol and mark the place to peirce with a pen. They use a large clamp to hold the skin then they just stab the needle through. They put in the ring or barbell for you so fast that you don't even see it. Then they clean you up again and cover it with gauze.
