Its most likely from jewelry that is too long or balls that are too big. GO see a piercer and just ask them to fit you for a bar and see if they have smaller ends. For the bottom ball, its a good idea to get a half dome or whats referred to as either a ufo or m&m end (I'm sure they have proper names :P) to reduce the friction in that area :)
