Yes, that means that recently you have been putting your phone in the charger to hard and your not cautious. By doing that the charger shapes the hole and makes it too big. Next time carefully insert the charger in your phone.
either the charger is not working well, or your phone is the problem. i would get a bettery replacement depending on how old the phone is, or go get it checked out at the mobile company u got it from they usually check it free :) take advantage

There are different things it may mean. My macbook said that it wasn't charging even though it was plugged in, and it turned out that the reason was because the charger was plugged in the wrong way :) (the charger should be facing out, away from the laptop)

read the label on your charger if it says 90v to 250V it will be ok it it says 120V it will not be ok
Just say to your friend that your phone is out of battery and you can't find the charger . If she or he calls you and says I thought your phone was out of charge and you couldn't find the charger just say that you found the charger. this actually happened to me too either you take out the battery and let it sit out for about 6 hours or you replace the battery.
