Nothing major. If you wait until your piercings [and each subsequent enlarging afterward] are healed before progressing, you don't skip sizes, and all of the other good tips that both common sense and the internet will tell you, the discomfort will be quite minimal. Nothing that should keep anyone from gauging up if they want to.
I personally go up by about four sizes each time, and the pain still isn't bad at all. Although they have ripped once, but I will reiterate that I went up by twice the recommended size, at once. Just use some common sense like cleaning, checking for infection/rips, etc, and all will be well.
Gauges HURT. It's just the stretching process, I used things around my house to stretch my earlobes to the size it is right now, which is 5/8ths. Depending on how much you stretch it everyday, then the pain can vary. if you go one size up every month, the pain should be very minimal. Don't stretch too fast, because then you get what I call "butt ears" Because it looks like an anal hole.
If you are getting a ear piercing it doesn't hurt at all but other kinds of piercings non your body hurt depending on where you get them!
