That varies from one book, play or movie to another. In romantic stories there are often just two main characters. Sometimes you can tell a play or movie's main characters by how many stars' names are printed above the title, but that sometimes depends on the celebrity of the stars more than on the structure of the story. The movie Miracle on 34th Street (1947) billed the two romantic leads above the title, but everyone thought the main character was Kris Kringle, played by Edmund Gwenn, who was clearly the principal character of the story even though Gwenn was considered only a featured player.
