That fact sheet was totally wrong... for males the healing time is 3-6 months, females 6 months to a year. I recently got mine done from a well established piercer and am very meticulous about avoiding infection. Hope you did further research outside of the above answer before you made your decision to pierce.
Just got mine done (professionally) and she said 6 weeks to fully heal (and to not let anyone contact it orally for that long)

Provided you haven't contracted an infection they should just heal over with no problem. If there is reddening and pain at the site, see a doctor.
There's no way to be sure honestly. Some people can have piercings for years that never fully heal to the point that they don't close. If you have other piercings that have healed to that point, you can use that as a gauge but otherwise, don't remove the jewelry unless you want to risk losing the piercing :)
You don't. That's just because most doctors are against piercings. You can always deny a ridiculous request like this and continue surgery with your nipple piercings in.

Cartilage piercings never close the surface tissue may shrink to obscure the opening but the hole in the cartilage is life long and will never heal up.
