The way you know is rejecting it gets red swollen and you feel pain. You can also see the jewelry pushing itself out and it may start to look crooked. Just keep it clean with warm water and sea salt water. Dont clean it to much. I only clean it once a day.
If you notice it rejecting, go to your piercing and see if anything can be done to keep it. You may be able to save it or you'll just get it removed and prevent more serious scarring.

It sounds to me like your talking about a dermal or a horizontal navel piercing which is a surface piercing. But from the sound of it it's a dermal piercing because I've never seen a surface piercing with 3 balls unless there 3 different piercing in a row.
