The PUK is stored by the network provider. You would need to contact them - and provide the requested security information, before they'll give you the code !
The PUK-code is (Personal Unblocking Key). It is NOT unique for Nokia phones. If a user enters the wrong PIN-code 3 times in a row, the telephone asks for the PUK-code as is supplied by the network operator for that telephone. Don't lose it! If the PUK-code is wrongly entered 10 times in a row the SIM-card gets locked up permanently.

Ok If you want your PUK code, you will have to call your service provider (etc. optus, virgin mobile) and they will transfer you to a specific number which will give you the information you need, then you will have to state your name and date of birth and then they will give you your PUK number

There's nothing Nokia or a service centre will be able to do. The PUK code is a function of the SIM and you will need to contact your network for the code. Some networks have a bit on their website to find your PUK code. If it's O2 UK you can get it from the website by going to help > unblock my mobile > enter your mobiel number. If you can't get it from a website, just ring up the network and they should give it to you without any problems. The network is the only place you will be able to get it from though.
A PUK code needs to be entered when you have had three tries and could not enter the correct sim pin. The PUK code will be written on your sim card
