To calculate percent usual body weight (%UBW) you just use the following formula:

%UBW = (100 x Actual Weight)/Usual body weight

For example, if you have a woman who normally weighs 180lbs, but due to trauma or surgery currently weighs only 162, her %UBW would be as follows:

%UBW = (100 x 162)/180 = 90%
