Slaves were treated extremely poorly. Most of them were kept in atrocious living conditions, and if the women were pregnant, they were forced to give birth on board and their babies would often die. Conditions were so cramped that on some ships, people were sandwiched together between boards, on top of each other. If anyone spoke their own language they would be severely beaten/punished. Thousands died in the middle passage, so their bodies were thrown in the sea.

The starting and ending points of the middle passage in that slaves spent most of the voyage below deck, manacled and then were sold. this happened in 1444 when the Portuguese sailors captured coastal Africans.
