It is much more common for the pregnancy to occur from failing to wear undergarments. Pregnancy is caused when semen reaches and fertilizes an egg, which is very unlikely to occur without vaginal penetration of some kind.

There is an apocryphal story of women who became pregnant when a bullet fired during the American Civil War passed through a man's testicle and lodged in a woman's womb. The baby was said to have been born normally and only found out about the incident when he had to have a bullet removed from his head as an adult.

This can't be independently verified. Women can become pregnant through artificial insemination, but this is very unlikely to happen without the knowledge of the woman.
