I may not be able to answer this question with the knowledge of a doctor, but I can lend you whatever insights I have as a former smoker. Smoking four cigs a day is not a terrible amount to be smoking. However, for me it started out with one cig at night to walk and relax and take it easy. The number of course slowly grew as my apathy did for my developing addiction. Man, cigs are gnarly, they sneak up on you. Plus I mean I smoked for 3 years or so, slowly growing in how much I smoked daily and I even went through a couple quitting bouts. But what happened is that smoking becomes less and less of a pleasure and an enjoyment, which is what it started out for me as, and more and more of a vice. So you know you may not feel or have too many physical problems with only smoking four cigs a day, but the problem is that a lot of the times that number grows and gets harder and harder to resist. Man, tabac is great and cigs are nice, but I learned its just better to say later to them.

Smoking one cigarette is not going to cause damage. You probably breathed in more harmful elements in the town you live in, to include vehicle exhaust fumes. That being said, continue cigarette use will cause damage, not only to your lungs, but many other parts of the body. Thus, I would strongly recommend that if that was your first cigarette, make it your last one.
Smoking ANY number of cigarettes at ANY time is bad for your health and it causes cancer and various other health problems.
