Lightning does indeed strike first, as thunder is the sound created by lightning. According to Wikipedia, lightning heats the air surrounding it by large quantities, which in turn causes the air to expand and create a "Sonic wave" which is similar to a sonic boom.

Sheet lightning and Heat lightning are very similar, if not the same. They form the same way, but might strike different. For instance, sheet lightning looks more like a sheet in the sky, (strange eh? :D) and doesn't normally strike down into the ground, and when you have heat lightning, you see differently.
