Microdermals should be surgically removed. Barbells can just be taken out, but they're horrible for surface piercings. But, back dimple piercings have a tendency to reject because of the friction and movement of that area. So, they may come out on their own or be pulled out before you decide that you want them removed.

Nope. Back dimple and hip piercings reject super quickly. Those areas move way too much for the piercing to heal. Also, theres a lot of contact with clothes which irritates the piercing and can get snagged. Really, they're a waste of money. You'll be super lucky if they last a year. You dont want to spend all of that money just to get scars :/ Don't even try using barbells or surface barbells. Theyre even worse for that area. They may be cheaper but you'll be left with bigger scars if they reject. They might be pretty and cute, but back dimple piercings are a terrible idea. Sorry :(
You'll have to either go back to your piercer/find a piercer to remove it or have a doctor remove it.

Its different for everyone but I'm gonna guess yes. The procedure takes a bit longer and theres a bigger affected area. For back dimple piercings, youre gonna want micro dermals, they'll last longer. Also, I'd feel terrible if I didnt warn you about these piercings. Just like hip piercings, they have a high rejection rate. Theres a lot of movement and contact in that area so they reject quickly. Most dont last a year. Not to mention that they'll most likely leave you with scars. In my opinion, its a waste of money to pay for such expensive piercings if they wont last :) Good luck
I have 4 oral piercings, and I smoke&toke. Nothing has ever went wrong. So do what you please. c: - Try to cut down on smoking during the healing period. It increase the time that it takes for your piercing to heal. Afterwards, do as you please but try your best to cut back during healing.

Microdermals are pretty safe. Never ever use a bar or even a surface bar for a surface piercing. Back dimples are EXTREMELY hard to keep. Theres a ton of movement and contact in that area. You'd have to put less pressure on your back when you sit, be careful while you get dressed, and not move a lot. Which is pretty tough. Back dimple and hip piercings are notorious for rejecting within a year. I would look into getting a dermal in a better place but hey, if you really want it I can't stop you.
