The honest answer is that it is impossible to count or even to estimate the number. Smoking is not a certifiable cause of death. 'Smoking related' conditions are multifactorial, i.e. they are caused by a combination of several or many contributing factors. For example, there are over sixty suspected contributing factors for lung cancer and the number of possible combinations of those factors is over 8 x 1018. So to claim that a death is 'caused' by one single factor is living in cloud cuckoo land.
Everyone dies eventually (even non-smokers!) but no-one is allowed to die from 'old age' or 'of natural causes' any longer. The zealous health lobby groups circle the corpse like vultures to claim the unfortunate victim for their cause. As you can imagine, several lobby groups will claim the same victim, with the result that the total number of claimed deaths greatly outnumbers the actual number of deaths!
