I have a Maestro debit card issued by the Credit Suisse bank, and when I was in the USA a year ago, the card worked in some stores but not in others. And unfortunately, I have no way of knowing WHY some stores accept it and others do not. But as for using it at an ATM machine there in the US, you can only use it if the Maestro symbol is on the ATM machine, and this usually is the case only if the ATM machine is at a large national bank that has multiple locations (and not at a small bank with only one location).

Not all Maestro cards have an issue number. Normally if no issue number is available, leave this section of the form blank and instead fill in the portion of the form that asks for a "card start date" or similar.
