It depends if it fell out and it's just been a few hours then you can gauge it back open. The middle of the hole seals up because your tongue is a muscle and the muscle goes back to it's original state as soon as the bar is removed from your tongue. If it's been a while then it's safe to get it re-pierced.
well i have not had my lip pierced but i have had the Labret which is very close to the lip and i have my tongue pierced you cannot feel your tongue being pierced, but its almost immpossible to eat for about a week . The labret hurt like hell though.
Yes but I wouldn't. Anytime you get a piercing there's a chance of infection and you really don't want an infection while you're pregnant.

My labret (usually said to be pretty much identical to a "monroe" pain wise) and my tongue were simple and painless, but I thought I was going to cry getting my nostril pierced. My sister fainted getting her philtrum (proper term for any upper lip piercing) but she was laughing and talking while getting her nostril pierced. There is no way to tell which will be worse for you, and a good piercer can make all the difference in the world.
