Can a doctor tell how far along pregnant you are without a sonogram?

A good Doctor should be able to tell by measuring your uterus. Any experienced midwife or Doctor (I have only used midwives myself) should know how to tell this by measuring and it has been very accurate in my experience. I believe they start at the pubic bone and measure up the uterus by feeling through the skin. The number of inches is how many weeks you are, if I remember correctly. This would not be easy for someone who does not know what they are feeling for, as I have had two children and cannot do it myself still.

Not necessarily a bad sign. You might just think you are further along than you really are and that's why a heartbeat cannot be detected YET!

I would say yes. They measure your stomach on the outside every visit to make sure your as far along as expected. They also measure the width of the baby and the sack during a sonogram. They should know how far along you are as long as you are going to your appointments regularly.
