If the dermal is of the standard design and happens to have holes in the feet, they will need to be removed by your piercer or other professional piercer who knows what they are doing to minimize the soft tissue damage during removal. The average GP doesn't know how this jewellery works and they tend to be way too rough and scalpel happy in an attempt to remove the dermals. This tends to lead to excessive scarring. I would suggest you talk to your GP and or OBGYN and get there opinion as to which way to go, leave them in as I have stated or remove them and have them replaced after the delivery date.
