Okay so this might be a spoiler so if you haven't finished the book, don't read any further!! Towards the end of the book, Percy, Annabeth and Grover go to Lord Hades in the Underworld. It turns out Hades has Percy's mother and offers her in exchange for the lightning bolt that Luke sneaked into Percy's backpack to get him into trouble. Percy refuses and takes out three magic pearls which were a gift from his father. Once shattered, each pearl will transport one person to where ever they want to go. His two friends take one each and Annabeth, Grover and Percy leave with the lightning bolt, leaving Percy's mother behind. This is what the oracle means by him failing to save what matters most in the end- failing to save his mother. In the end however, Hades realises that Percy is not at fault and sends Sally Jackson, his mother, back to her home on Earth. So Percy didn't manage to save her, but Hades did.
In the Percy Jackson and the Olympians series, Persephone would sneak lovers in and out of Underworld. They would break the pearl in order to travel between the world and the underworld. Percy needed the pearls to help save his mother from Hades.
In the book The Lightning Thief, the kids don't actually need to search for the pearls; a naiad gives them to Percy. But in the movie, they are called Persephone's Pearls, and they are hidden in monuments across the country. When you crush the pearl, it transports you from the underworld to the normal world.
