Being a blood brother(or sister I suppose) was an old tradition of confirming deep friendship by mixing blood. In its most typical form the persons would each cut their palm and then shake hands - but any similar ritual would do. With what we know about blood borne diseases today, this isn't really advisable any more.

To become blood sisters, two people typically make a pact or ritual to symbolize their bond, often by pricking their fingers and mixing their blood. This act is usually done as a sign of loyalty, friendship, and commitment to each other. It is a personal and meaningful ritual that solidifies a strong bond between the individuals involved.

You Prick Yourself With A Needle On Your Hand Then Shake Hands. Or jump of a swing and let your friend pick at a scab or something.
