I don't know what you mean by his initials being GH. That makes no sense whatsoever. He attributed it to years of smoking, which, I guess, you could say would make it possibly inevitable. Lots of smokers die from something unrelated. You are right when you say there's a risk. He was also a chain smoker, who had stopped at one point, then picked it up again. Some of it habit, some of it stress-related, who knows?
Exactly, who knows? It's pure speculation. Cancer is a multi-factorial disease i.e. there is not a single cause but a number of contributing factors, some known, some suspected and, in all probability, some unknown. There is a strong hereditary link for cancer and both his parents died of cancer. Also, cancers in other parts of the body can spread to the lungs and George had earlier recovered from throat cancer. But again, it's pure speculation.
