The concept of the song, and the entire album, is that when you die, death comes for you in the form of your favorite memory. For the main character, the Patient, his favorite memory was watching a parade with his dad. When he dies, it becomes the Black Parade.

If I remember correctly it just kinda travels down the piano while going upwards after every couple of notes. It starts at a high note.
