In looking at the English translation of the lyrics, I am pretty certain that the song is referring to the last days in the life of Enrico Caruso, a great Opera singer of the late 19th and early 20th century. The singer suffered an untimely death at 48. He went back home to Naples from New York City to die. I would imagine that this song is telling of him singing to his beloved wife with the knowledge that he will soon pass on and leave her behind. If you look up the English translation and compare it with documentation of Caruso's last days, the gaps will fill themselves in.
