Katerina is not the original Petrova, she is also a doppleganger. Klaus' mother was a witch and she was the one who created the first vampire, Mikael. Mikael then turned his children, including Klaus, into vampires. In order to turn them he spiked their wine with the blood of a girl named Tatia. Tatia was the original Petrova, the one that Katerina and Elena are dopplegangers of. Klaus' mother had an affair with a werewolf, resulting in Klaus being a hybrid. When Klaus made his first kill as a vampire, it triggered his werewolf gene. His mother sealed the werewolf part of him by using a spell. Inorder for Klaus to become a full hybrid and unlock his power, he had to kill the doppleganger. The reason his mother placed the curse that way was because in order for Klaus to create more hybrids, he needed the dopplegangers blood. If the doppleganger had been sacrificed to break the curse, then he wouldn't be able to sire any hybrids. Klaus didn't know about that though, he just knew that he had to kill the doppleganger I'm order to break the curse. He was going to use Katerina to break it the first time, but she escaped and had herself turned into a vampire so that he could no longer use her blood. In retaliation Klaus killed all of her family, except for her daughter, who was given up for adoption because she was born out of wedlock, resulting in Katerina and Elena being related.
