You should have a code in your owners manual. If you can't find it, call your dealership and ask them to look it up for you.
In a 2008 Yaris, you have to hold down SEEK and press 1.
This should appear: _ /_ /_
Say the 3 digit code was 987, you would need to press the button 1 ten times, the button 2 nine times and the button 3 eight times. (As the numbers begin from 0-9)
Your screen should now say: 9/8/7
Hold down scan to finish.
Or if your model is different, call your dealership service department and they will tell you how to do it.

Well remove the radio and there is a master reset on the radio. But if you just remove the positive terminal from the battery it should reset it. And you don't have to remove the radio to do that

My car does this all the time. I am not positive but I was told it has something to do with a glitch in my security/alarm system. You can either manually lock your doors. Count to twenty and then unlock the door with your key which is supposed to reset the security or you can reset the battery.

Leave the radio on for 1 hour and it will reset and then accept the 5 digit security code.

What worked for me was to unplug the antenna in the back of the radio and then enter the security code. Once the security code is entered, you can plug the antenna connector back into the radio.
